You may complete this application electronically but you cannot store it or submit it online.
It is recommended that you download the application form and save it to your hard drive. Then, enter the required information for your building and save the file with an appropriate file name for that property.
When the application is complete, print it, sign it where indicated, include required attachments, and mail to the address on the application.
